We’ve reimagined our 60-bed facility from the ground up — blending small-town warmth with modern design, upgraded amenities, and advanced clinical resources. Located next to a progressive care hospital, we provide peace of mind knowing professional medical support is always close at hand.

Appleton City Manor offers both long-term and short-term skilled nursing care, as well as comprehensive physical, occupational, and speech therapy services. Residents enjoy personalized comforts including:

Our Services

    • Nursing and Therapy work closely as a team to ensure your loved one’s goals are met and continue to be carried out.

    • Family Nurse Practitioner on-site daily.

    • Physician on-site weekly.

    • Nurse Educator trains staff and also serves as a Diabetic Educator.

    • We offer assistance with insurance issues.

    • We work closely with your hospice provider to develop a compassionate team approach.

    • Respite care available for caregivers who need temporary relief—whether for a few days or longer.

    • Weekly visits from a Nurse Practitioner with Wound Care Plus.

    • They assess, treat, and follow up with the nursing team to ensure optimal recovery.

    • We provide Occupational, Physical, and Speech Therapy on-site as needed
